What a GLP-1 costs without insurance, by route

Every price here is dated and sourced, because they move constantly. The number that surprises most people is not the drug — it is what sits on top of it.

What a telehealth programme adds

This is the part that catches people. Advertised prices are usually medication only.

Hims lists the Wegovy pill from $149 and the Zepbound vial from $299 — with a required membership of $39 the first month, then $149 a month, billed separately, which their own footnote says "does not include or guarantee a prescription."

Same pill. The membership buys clinician access, messaging and follow-up — real things, worth paying for if you use them. It should be a choice made with both numbers visible, and one of them is usually in small print.

Found is structured differently again: $169 a month cash — but that is a twelve-month prepayment, month-to-month is $289, and it is billed per 28-day cycle, which is 13 payments a year, not 12.

Before you pay any cash price

Check coverage first. It beats every number above when it applies.

  1. Call the number on your insurance card and ask specifically: is tirzepatide covered? Is semaglutide? Under what diagnosis?
  2. Ask about a sleep apnea diagnosis. Zepbound is approved for obstructive sleep apnea in adults with obesity, and plans excluding weight-loss drugs sometimes cover a diagnosed sleep disorder. More on that here.
  3. Ask about prior authorisation. Many plans cover these only after documented attempts at other approaches.
  4. Ask whether an appeal is possible if the first answer is no. Often it is.

The cheap option that is not one

Compounded semaglutide and tirzepatide undercut everything above. They are not FDA-approved products, the approved labelling does not cover them, and the FDA has logged over 1,700 adverse event reports including ten-fold dosing errors from unit-conversion mistakes. The detail is here.
