Zepbound for sleep apnea: what the approval actually covers

This is the one approval that separates Zepbound from Wegovy, and the one most likely to change what insurance will pay for.

What the approval says

Zepbound is approved for obstructive sleep apnea in adults with obesity. It is the only GLP-1 medicine that carries this, and it is the clearest difference between Zepbound and Wegovy.

The doses are not the same as for weight alone. The label's maintenance options:

Everyone still starts at 2.5 mg for four weeks and steps up in 2.5 mg increments with at least four weeks on each dose. The maximum for every indication is 15 mg.

Why this matters more than it sounds

Insurance. A plan that flatly excludes weight-loss medication may still cover a drug prescribed against a diagnosed sleep disorder. Same drug, same person, different answer — because coverage follows the diagnosis, not the molecule. If you have a sleep study showing obstructive sleep apnea, that is worth raising with both your prescriber and your plan before you accept a cash price.

It is a real clinical indication, not a marketing angle. Obstructive sleep apnea in the context of obesity is a serious condition, and this approval exists because the drug was studied against it.

What it does not do

It also does not turn Zepbound into a different drug. The boxed warning about thyroid C-cell tumors applies, as do the contraindications: medullary thyroid carcinoma, MEN 2, serious hypersensitivity. A sleep apnea diagnosis does not create an exception to any of those.

Questions worth asking

  1. Do I have a sleep study confirming obstructive sleep apnea, or a suspicion of it?
  2. Does my plan cover tirzepatide for that diagnosis, even if it excludes weight-loss drugs?
  3. Who is managing the sleep apnea, and do they know I am starting this?
  4. When is the CPAP reassessed?
