Zepbound vs Mounjaro: the same drug, approved for different things

So what actually differs

What the label says it is for. That single fact drives everything else.

Insurance. Plans that exclude weight-loss drugs often cover diabetes drugs. The same person, the same molecule, a completely different answer depending on which name is on the prescription and which diagnosis is attached.

Price. Not because one is more expensive to make, but because the savings programmes and coverage pathways differ. On 23 September 2026, Hims listed the Zepbound vial from $299 a month and Mounjaro at $1,899 — the same drug, a sixfold difference, entirely from which route you are on.

Whether your use is on-label. Taking Mounjaro to lose weight is off-label. That is legal and common, and it also means the approval studies behind your prescription were not run for your reason.

What does not differ

Side effects, warnings and who must not take it. Everything on the Zepbound side effects page applies identically to Mounjaro. Same nausea rates, same boxed warning, same absolute bar for anyone with a history of medullary thyroid carcinoma or MEN 2.

The honest summary

If you have type 2 diabetes, Mounjaro is the approved product. If your reason is weight, Zepbound is, and it is the one with the sleep apnea approval as well. Which you can actually get is usually decided by your insurer, not your preference.
